LASER SURFACE CLEANING RENTAL AGREEMENT Pulse 200W Hard Case 120/220V Laser Cleaner Effective Date: Date of signed booking. Lessor: Laser Surface Cleaning, 950 N Richmond Ln, Palmer Alaska, AK 99645 Lessee: Individual/entity listed at rental booking execution 1. Equipment & Accessories Includes one Pulse 200W Hard Case Laser Cleaner, goggles, and gloves. Optional Forced Air Breathing Apparatus (FABA) available at additional cost. 2. Rental Rates Period Equipment Only With FABA Daily $329 $379 Weekly $1,974 $2,274 Monthly $7,238 $8,338 Full prepayment required. Refundable deposits: $500 (daily), $1500 (weekly), $2500 (monthly). Taxes not included. 3. Delivery & Pickup Fees: $35 (Mat-Su), $50 (Anchorage), $150+ (remote). Lessee responsible for loading/unloading and accessibility. 4. Lessee Responsibilities Provide 120/220V power. Use Equipment only as trained and within Alaska unless approved. Lessee is liable for misuse, damage, unauthorized use, or illegal activity. Operators must be 18+. Any demonstration by Lessor to show Lessee how to operate machine does not imply or induce Lessor liability or remove any liabilities from Lessee as outlined in this Agreement. 5. Extensions & Late Fees Extensions must be requested 2+ hours in advance. Period Per Hour 24-Hour Cap Extension $75 $329 Late Return $95 $340 Unapproved returns over 24 hours may result in repossession at Lessee’s cost. 6. Safety Class IV laser risks include eye and skin injury. Mandatory use of provided PPE. Lessee must maintain a safe work area, follow all instructions, and restrict access to trained users only. No damaged cords or wet hands near power source. 7. Damage, Repair & Return Lessee liable for damage beyond normal wear. Unauthorized repairs are prohibited. Equipment must be inspected upon delivery and return. Damage charges may be deducted from the deposit. 8. FABA Use Optional. Lessee must follow instructions. Lessor not liable for health issues or misuse. 9. Termination & Renewal Lessor may terminate for breach. Renewals require written agreement prior to end of term. 10.
